Do you have a chronic medical condition? Perhaps you require treatment from 3 or more health providers.

If this is the case, you may be eligible for a GP management plan (formerly known as EPC) and/or Team Care Arrangement.

Discuss with your GP to determine your eligibility. If approved, Medicare can subsidise $56 towards up to 5 of your physiotherapy appointments.
